The following list of aids comprise screening measures which may be printed off and given to your patients to be completed. Some of the assessment sheets come with prescribing notes that contain additional useful information for clinicians.
A Self-report Questionnaire designed for the patient. This is a state depression measure, originally designed for assessing depression in medically ill subjects (and thus excluding somatic items), but which has also demonstrated excellent psychometric properties in general practice and psychiatric settings. Responses are scored as follows: Very True (3), Moderately True (2), Slightly True (1) and Not True (0). A total score of 9 or more is indicative of depression - although duration and impairment criteria need to be factored into any decision about 'clinical depression'. The Black Dog Institute research group has developed its own Bipolar Disorder Screening Tool, which again can be used as a self-rating tool that patients complete themselves or with a clinician. The tool can be done online, or, if you prefer, you may download the tool, print it out and use it freely in your own clinical setting.

The Temperament and Personality (T&P) questionnaire identifies a number of personality styles that have been held to predispose to non-melancholic depression (in particular) and that correspond with four of the five factors identified in the well known Five Factor Model (FFM) of normative personality.

This 10 item self report measure is designed to screen women for symptoms of emotional distress during pregnancy and the postnatal period. It was developed by L. Murray and J. L. Cox, 1990. It is also known as The Edinburgh Postnatal Depression Scale (EPDS) (J L Cox, J M. Holden, R Sagovsky – 1987).
